Have Your Information Ready

When you’re ready to start the Mainspring Recovery admissions process, it’s important to have certain information ready. Having this information on hand will help us determine if we can meet your needs and will also speed up the admissions process. Here is a list of information that may be requested during the admissions process:

  • Personal contact information – We will need to have your name, phone number, address, and emergency contact information.
  • Insurance information – We accept some insurance providers, so we will need to know your insurance provider, policy number, and group number.
  • Medical information – We will need to know if you have any pre-existing medical conditions, any current medications you are taking, and the names and contact information of your primary care physician and any specialists.
  • Legal information – If you have a history of legal problems related to your addiction, we will need to know about it. This can include arrests, charges, or convictions related to substance abuse.
  • Substance use history – We will need to know what substances you have been using and for how long, as well as any previous treatment you have received for substance abuse.

We may also ask for a release of information form, allowing us to communicate with your primary care physician or other treatment providers.
